Information you give us
When you fill out a form or request an appointment, we collect what you enter: your name, phone number, email address, and the general goal or interest you select (for example, "more energy day to day"). If you continue to booking, we also collect your requested appointment details.
Information collected automatically
We use first-party analytics to understand how our pages and ads perform. This measures things like which page you visited, which links or buttons you clicked, the page that referred you, campaign parameters in the link (such as utm_source), your device's general type, and the time of the visit. We do not record the contents of what you type into forms in analytics. This analytics is on by default; you can decline or manage it anytime (see Cookies and tracking), and if you decline, this measurement stops.
Cookies and local storage
We use a small amount of browser storage to run the site: to remember your privacy choice, and, if you move from a page to booking, to carry your entered details forward so you do not have to retype them. Analytics storage is on by default and stops if you decline. See Cookies and tracking below.
How we use your information
- To respond to your request, confirm your appointment, and arrange any payment.
- To follow up with you about your inquiry by phone, text, or email.
- To provide, secure, and improve these pages.
- To measure how well our pages and advertising perform so we can improve them (analytics is on by default; you can decline anytime).
- To meet legal, safety, and record-keeping obligations.
How we share your information
We share your information only as described here. We do not sell your personal information.
- With Envista Medical. The practice receives your request so its team can contact you, schedule your visit, and provide care.
- With service providers who work for us. Companies that help us operate these pages handle your information under our instructions and only to do their job. They fall into these categories: a customer-relationship and appointment platform, a workflow-automation and messaging provider, website hosting and security, and a first-party analytics service.
- With advertising platforms, in a limited and de-identified way. See Advertising measurement for exactly what is and is not sent.
- For legal and safety reasons. If required by law, or to protect the rights, safety, and property of you, the practice, or others.
- In a business transfer. If the practice or its operations are reorganized or transferred, information may move as part of that, subject to this policy.
The specific providers we use may change over time as we improve the service. When they do, we hold the replacement to the same standard.
Cookies and tracking
You are in control of non-essential tracking. When you first visit, a banner explains this and lets you Decline or Manage your choices by category. Analytics is on by default and stops if you decline; advertising stays off unless you turn it on. You can change your choice at any time using the link in our page footers.

Advertising measurement
We advertise these services online and want to know which ads lead to real appointment requests, without exposing your health interests to advertising companies. To do this:
- When you submit a request, our own server may send a de-identified conversion signal to an advertising platform (for example, Meta) so we can measure that an ad worked. Where identifiers such as an email or phone number are used for matching, they are cryptographically hashed first, not sent in the clear.
- We do not run a browser advertising pixel that ties you to a health-condition page, and we work to keep the specific health condition of a page out of what we share for measurement, limiting it to what is needed to confirm that a conversion happened.
- This server-side measurement happens when you submit a request and is separate from the cookie choices above; it does not place advertising cookies in your browser.
This approach is deliberate: it lets us measure advertising responsibly while protecting your health interests.
Calls and text messages
If you provide your phone number and submit a request, you consent to receive notifications, marketing, and promotional text messages and calls from Envista Medical about your inquiry and its services. Message frequency may vary. Message and data rates may apply. You can opt out of text messages at any time by replying STOP, and reply HELP for help. Opting out of marketing messages does not stop messages needed to service an appointment you have requested.
Your choices and rights
- Tracking: accept, decline, or change your cookie and analytics choices anytime with the Privacy choices link in the footer.
- Messages: reply STOP to opt out of texts; ask us to stop marketing calls or emails.
- Access, correction, and deletion: you can ask us for a copy of the personal information we hold from these pages, or ask us to correct or delete it, by contacting us below.
- California residents: subject to applicable law, you may have rights to know, delete, and correct your personal information, and to not be discriminated against for exercising them. We do not sell your personal information. Contact us to make a request.
Security and retention
We use reasonable administrative and technical safeguards to protect the information collected through these pages, and we keep it only as long as needed for the purposes above or as the law requires. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.

These pages are for general information and to help you request an appointment. They are not medical advice and do not create a doctor-patient relationship. For care decisions, speak with a qualified provider.
Children
These pages are intended for adults and are not directed to children under 18. We do not knowingly collect information from children.
Other sites
If a link takes you to another website (for example, the main envistamedical.com site or a booking tool), that site's own privacy practices apply there.
